5.C.2. The Three Copies of the Voter Registration Form

The voter registration forms and the use of the voter registration feature on the state's e-poll book may vary according to jurisdiction.

This is meant as an overview and is not meant to contradict the local practices of Election Authority.

For Grace Period Registration, the Election Judge enters the information on to the paper copy registration record card, then enters the same information into the state's online voter registration form as part of the online e-poll book.

Note: It doesn't matter if the information is copied from the paper copy to the online form or vice versa. As long as the information matches.

The paper copy is handed to or shown to the registrant to confirm the information is accurate.

The Election Judge should then ask the registrant to read and sign the Affidavit of Registration printed on the form.

The Election Judge may also want to inform the registrant that their signature will be the signature used for comparison for future voting.

The Election Judge then separates the duplicate copy of the registration record card and hands the form's duplicate to the voter as proof of registration.

The three copies:

  • The online form is used to update the state database.
  • The paper copy is returned to the Election Authority.
  • The duplicate of the paper copy is given to the voter as proof of registration.
